Instructions:

You can fill out the following forms and submit them electronically via email to Santé before your visit. Please make sure that you have Adobe Acrobat version 7.0 or higher for best results. If you need the latest version, please click here to download.

Download the forms you need by Right-Clicking the links below, and choosing Save As...

After saving the PDF file to a location on your computer, you can open it with Acrobat and simply type your information into the form directly and click checkboxes to indicate your selections. Once done, you can save the file and email it to Santé.
