High-Risk Cardiovascular Patient Management Program

Evaluating high-risk patients, or caring for those with know cardiovascular disease, can be tricky, time consuming and often requires equipment and technical expertise that many practices are not equipped for. Santé Personalized Cardiology has an answer that will not simply lead to more unhelpful diagnostic tests but will use state-of-the-art technology, aggressive risk factor modification, and old-fashioned personal attention to reduce the risk for subsequent devastating cardiovascular events.
Patients with known cardiovascular disease or multiple cardiac risk-factors are evaluated in a private, concierge-setting by Dr. John McB. Hodgson, former Professor of Medicine at Case-Western Reserve and interventional cardiologist for the past 25 years. Using the latest imaging techniques to actually look at the coronary vessel wall, Dr. Hodgson will evaluate each patient, perform the appropriate imaging tests, and set evidence-based management protocols using our patented Guardian VPM™ (virtual patient management) system to ensure continuity of care with the primary care physician.
Santé’s High-Risk Cardiovascular Health Management Program includes the following:
- Assessment of overall health, including detailed cardiovascular and complete physical exam
- Screening and risk assessment for heart and cardiovascular disease
- VasoPat Endothelial Function Test (prognostic marker of atherosclerosis), initial exam
- Electrocardiogram (EKG)
- Calcium-score CT heart screening (if over 40 years old)
- Ankle Brachial Index calculation
- Exercise and fitness assessment
- Diet and nutrition assessment; two month supply of customized supplements from NutriScan™
- Screening and risk-assessment for common cancers, including colorectal, prostate and breast
- Full laboratory studies necessary to obtain a complete profile of major organ systems, cardiovascular risk factors and VAP cholesterol
- Year-long use of our patented Guardian VPM™ (virtual patient management) system for managing risk-factors
- Year-long access to web-based wellness toolkit including diet and nutrition programs that can be customized to fit individual patient-needs including cognitive behavior therapy-based programs for smoking cessation, weight loss and exercise
- 24/7 telephone and e-mail access to our physician and associated staff
- Preferred status, on-line scheduling (same or next business day appointments); home or office visits available as needed
- The cost for the High-Risk Health Management Program is $1,750 initially, then $500 quarterly. Out of network insurance claims will be filed for all testing.
