Risk-Factor Management Program

Managing patients with multiple or difficult to control risk factors can be time consuming and often requires subspecialty knowledge that many practices are not equipped for. Santé Personalized Cardiology wants to partner with you in caring for these patients.

Patients with risk factors are evaluated by Dr. John McB. Hodgson, former Professor of Medicine at Case Western Reserve University and interventional cardiologist for the past 25 years. Dr. Hodgson will evaluate each patient, determine the appropriate course of action, and set evidence-based management protocols using our patented GuardianVPM™ (virtual patient management) system to ensure continuity of care. As a partner physician, you will have access to the VPM system.

• Assessment of overall physical health with focused cardiovascular exam
• Screening and Framingham risk assessment for heart and cardiovascular disease
• Electrocardiogram (EKG) if not already performed
• Ankle Brachial Index calculation
• VasoPat Endothelial Function Test (earliest marker of atherosclerosis)
• Exercise and fitness assessment
• Diet and nutrition assessment
• Trial membership to use of our patented GuardianVPM™ (virtual patient management) system for managing risk-factors
• E-Mail access to our staff
• Online scheduling
• Year-long access to web-based wellness toolkit including diet and nutrition programs that can be customized to fit individual patient-needs including cognitive behavior therapy-based programs for smoking cessation, weight loss and exercise
  Follow up visit for report review.
• Detailed report and recommendations to the referring partner physician
• The cost for the Risk-factor Management Program is $450 initially, then $175 quarterly and can be bundled or unbundled with our other concierge-cardiology programs. Out of network insurance claims will be filed for all visits and testing not listed above.
